If you believe you have a malpractice claim against an Oregon State Bar member, or you are an attorney representing a client with a claim against an OSB member, please complete this form to receive instructions on how to report a claim and a Claim Initiation Form to complete and submit to the PLF. The instructions on initiating a claim and the Claim Initiation Form will be sent to the email address provided below.

IMPORTANT NOTICE

There are legal time deadlines for filing lawsuits (called statutes of limitations). Your request for a Claim Initiation Form and/or submission of a Claim Initiation Form to the PLF does not stop these deadlines and they will continue to run. If one of these deadlines passes, you may not be able to make a claim or sue the legal practitioner. We recommend that you consult with an attorney about legal deadlines and other rights. The PLF cannot give you any legal advice regarding these time deadlines or any other matter.
